The Lagos State Government has ordered a full-scale investigation into an alleged case of medical negligence following the tragic death of Nkanu, the son of celebrated Nigerian author, Chimamanda Ngozi Adichie, after a medical incident at a private hospital in Lagos on January 6.

In an official statement, the state government expressed profound sorrow over the incident, describing the death of a child as a devastating and irreparable loss. It conveyed its deepest condolences to Ms. Adichie and her family, assuring them of the government’s concern and commitment to justice.

The government reiterated that it places the highest premium on human life and maintains a zero-tolerance policy for medical negligence or unprofessional conduct in any health facility operating within Lagos State.

Following directives from Governor Babajide Sanwo-Olu, the Health Facility Monitoring and Accreditation Agency (HEFAMAA) has been mandated to immediately carry out a comprehensive, independent, and transparent investigation into the circumstances surrounding the incident. The probe is expected to uncover both the immediate and underlying causes of the death.

HEFAMAA has since commenced investigations and has already visited the healthcare facility involved. The agency will conduct a detailed review of all allegations, reports, and medical procedures connected to the case.

According to the statement, HEFAMAA—established by law to regulate, monitor, and accredit public and private health facilities in the state—routinely investigates cases of alleged medical negligence and unethical practices to ensure patient safety, professional standards, and quality healthcare delivery.

The investigation will be carried out in collaboration with the Medical and Dental Council of Nigeria (MDCN) and other relevant regulatory bodies to ensure a credible and professional process.

Authorities said the probe will assess compliance with approved clinical protocols, professional conduct, patient safety standards, and the roles and responsibilities of all parties involved. The findings, the government assured, will be made public upon conclusion in the interest of transparency and accountability.
The Lagos State Government warned that any individual or institution found culpable of negligence, misconduct, or regulatory violations will face appropriate sanctions in line with existing laws.

While the investigation is ongoing, the government urged the public to remain calm and refrain from speculation, stressing that due process will be strictly followed to ensure justice.

The statement, signed by Dr. (Mrs.) Kemi Ogunyemi, Special Adviser to the Governor on Health, reaffirmed Lagos State’s commitment to protecting the health and rights of residents and strengthening oversight of medical practice to prevent a recurrence of such tragic incidents.
Once again, the government extended its heartfelt condolences to Ms. Chimamanda Ngozi Adichie and her family.
